Apply for a Gift

Apply for a Grant

Hello and welcome to Rocky’s House! The Rocky’s House organization is your partner in providing financial assistance and services to families with children who are critically ill or in critical care who are experiencing prolonged inpatient hospitalizations. Rocky’s House is honored to review the families application and extend its resources while children and families are focusing on fighting for their health. Our founders know the impacts and worries your family is experiencing - and it is our goal to help sustain the family’s housing needs and provide a stable environment while the child/patient is receiving medical care and families are advocating for their loved one.

All applications are to be submitted online via our web-based application system by way of the hospital admin/designated social work team lead & partner.

Please note the below qualification factors to the gift/grant award program:

  • The family of the child/patient must work through the designated hospital partner/social work team to complete the application process & for any follow up communication.

  • The child/patient must be U.S. Citizen (parents and guardians do not)

  • The total income of the family or guardian(s) of the child/patient cannot exceed 120% of Area Median Income (“AMI”) as determined annually by the Department Housing and Urban Development (“HUD”). The Rocky’s House Board of Directors (BOD) reserves the right to review any application with regard to this requirement and may vote to waive on case-by-case basis. (Rocky’s House will provide guidance to the social work team on what these limits are based on the Applicants state and county of residence).

  • The child/patient must be 18 years of age or under to qualify for a gift/grant.

  • The child/patient must currently be inpatient at a participating partner hospital system.

  • The admittance of the child/patient and family to inpatient care must be >45 days before application can be executed and submitted. (the >45day requirement must be consecutive days, and not in aggregate to a calendar year or other and the child/patients’ condition must meet the healthcare providers definition of having received critical care or been critically ill for the majority of the inpatient stay).

  • The gift/grant levels and $ amount awarded to an applicant/family, can vary from < than or = to $2,500, to a maximum of $7,500 based on need – as determined by the application information and gift/grant committee. The gift/grant may be paid out over multiple months.

  • A family that has received a gift/grant under their original application can not receive a subsequent gift/grant for at least 12 months after its first award and must meet all the same requirements above.

  • Maximum lifetime gift(s)/grant(s) to applicants/families are capped to $15,000.

To begin an application, please work with the social work team member to visit our website click on "Applications" section and select the correct partner hospital application. Hospital Social Work will enter their designated passcode and select " Application". The Social Work team or medical professionals will be required to fill out the initial section completely before moving to the "Family/Guardian Applicant Information" section. The application will not be eligible for submission until all required sections are complete, & all necessary supporting documentation has been uploaded, and that the social work team has certified the accuracy of the information presented.

Once the "Family/Guardian Applicant Information" section is complete, a link will be sent to the Family/Guardian Applicant to securely upload supporting documentation.

To summarize, the steps for completing the Application are:

The hospital Social Work team begins application with Family/Guardian in the hospital by accessing “Applications” on website.

Social Work and/or Medical Professionals must fill in first part of the application

After the application is submitted, the system will generate an email with a secure link to the applicant family/guardians email address where the family/guardians can securely upload supporting documentation.

Social Work will begin application via their hospital system specific application & enter passcode

The Applicant family/guardian must fill in the remainder of the application and hit “submit".

Once the supporting documentation is uploaded the application is then considered “fully submitted” and ready for review by Rocky’s House award gift/grant team.

Applications are on a rolling basis and subject to availability of funds. Rocky’s House makes awards 12 times each year (every ~30 days) and awards are gifted/granted the middle of each month. Delivery time of the gift/grant will depend on if the applicant selects wire/direct deposit or mail. Rocky’s House will determine a maximum funding amount to the hospital system on a quarterly and yearly basis and subject to change due to available funds.

If you have any questions as you fill out the application, please send an email to RHSecretary@Rockys-House.org.

All applications must be submitted by a social worker at one of the organizations partner children's hospitals.
